A request for the consideration of a patient’s rights must include the following:
- the patient’s full name, residential address, and contact information,
- a description of the alleged violation of the patient’s rights,
- information about the involved healthcare professionals or associates,
- information about any other persons involved,
- the time and place of the alleged violation of the patient’s rights,
- any potential consequences resulting from the alleged violation, and
- a possible proposal for resolving the dispute.
If the written request does not clearly identify the applicant, or if the request is offensive or submitted too late, it will not be considered. If the written request does not contain all the necessary elements for review, the competent person will invite the patient to complete it within a specified period. If the patient fails to supplement the request within the given period, it shall be considered withdrawn.
