Patients and other individuals whose personal data are processed by the Novo mesto Health Center may request access to their personal data using the form available HERE. Please complete the form correctly and send it to the following address:
Novo mesto Health Center Kandijska cesta 4, 8000 Novo mesto (to the appropriate department or clinic). You may also request your personal data by submitting a hand-signed written request, which should be sent to the above address. Please ensure that you include all the necessary information as indicated in the form.
If you are requesting medical documentation of another person, please attach an authorization or a statement confirming that you are legally entitled to access such documentation.
